Mail / Fax Order Form Download

I understand that some people would rather not use a credit card on-line, so I will accept orders by mail or fax with payment by credit card, check or money order. However, I am not set up to take orders by phone. Should you reconsider and decide to order on-line using PayPal, you can be assured that PayPal is a very secure payment system, which uses encryption technology to protect your personal information and financial account information.

Mail/fax orders will only be accepted from US customers.
International customers (including Canada), please place your order on-line at Materials and Tools for Your Telephone Wiring Project.

To order by Mail or Fax:

Select a form below for your order.
Right click on the icon for the form, select "Save target as...", and save the file to a location of your choice.
Complete form:
- For the MS Excel file:
  - Open the file and fill in the form on your computer.
  - Print the completed form.
- For the PDF file:
  - Print the form using Adobe Reader.
  - Fill in the form by hand - please print neatly.
Send your order:
- FAX to: 866-405-8935 (toll free)
  - Payment must be by credit card.
  - Make sure to provide the CVV number and sign the order form.
  - Credit card charges will be listed on your statement as CEWIM LLC.

  - Checks and Money Orders must be payable to CEWIM, LLC.
  - If paying by credit card, make sure to provide the CVV number and sign the order form.

Please note: Some recently added items may not be listed on the order forms. If you want to order an item not listed on the order form, please use the on-line order process or contact me by email (see contact info below) for instructions.

MS Excel Form Easiest to complete. Requires MS Excel. Automatically performs all calculations.	PDF Form PDF format file - to be completed manually. Recommend you only use this option if you do not have MS Excel. Requires Adobe Reader. Click here to get a free copy of Adobe Reader. Get Adobe Reader If you don't have AdobeReader Version 8, I would recommend downloading the new version.
Note: As an additional security measure, these forms are accessed using SSL security.

Things to NOT do!

Don't send credit card information in an email - email is not secure.
Don't send an order by fax and indicate payment by check or money order.
Your order will not be shipped until payment is received and may be delayed if the payment is received without an order form.
Don't send an order by fax or mail without an email address and phone number.
Your order may be delayed if there is a problem with your order and I can't contact you by phone or email.

Don't enclose cash with your order by mail!

I trust you and I trust me,
but between you and me there are a lot of people I don't trust!

Faxed orders must be paid by credit card and will be processed and shipped as soon as possible after receipt. If you include an email address on your fax order, you will be sent an email acknowledging your order and you ill also be notified by email when your order has been shipped.

When you order by mail, you may charge it on a credit card or pay by USPS Money Order (preferred), Bank Money Order, Cashier’s Checks, Certified Check, or personal check. Mail orders paid by credit card or USPS Money Order or Bank Money Order will be processed and shipped as soon as possible after receipt. Mail orders paid by personal check will be held for check clearance for a minimum of four banking days before being processed and shipped. Mail orders paid by money orders issued by other institutions, like grocery stores and convenience stores, will be held for clearance, just like personal checks.

Note: "Personal Money Orders" and "Certified Checks", although obtained from a bank, are legally personal checks and will be held for clearance just like any other personal check. If you are getting a "money order" from a bank and you want your order shipped without waiting for check clearance, make sure to get a "Bank Money Order" or "Cashier’s Check", not a "Personal Money Order" or "Certified Check".

If you send an email indicating you have mailed an order, I will notify you if the order is not received within 14 days. If you provide an email address in your order, you will also be notified by email when your order has been shipped.
